Entry Deadlines [1] Friday 19 February 2010 [2] Friday 26 February 2010 All fully completed entry forms with payment and/or breed surveys with payment MUST be received on or before this date. Late entries will be accepted until this date, except for breed surveys. The entry fee surcharge of 100% per dog has been waved. You will receive a FREE show catalog per entered dog if you submitted your completed entry form and payment on or before the above two dates.

a) You must mail copies of the dog’s registration, pedigree and other applicable documents together with your check payment or money order and your signed entry form on or before the above dates.
b) All payments must be in US Dollars.
c) The original documents must be presented to the trial/show secretary at check-in.
d) It is possible to provide the name of the dog handler plus signature during show check-in.

INCOMPLETE ENTRIES, ENTRIES THAT ARE NOT SIGNED, AND ENTRIES WITHOUT PROPER DOCUMENTATION AND/OR PAYMENT WILL NOT BE ACCEPTED.

Mail completed entries, fees, and copies of the required documents to Donna Agee (trial/show secretary). Entries that are not received on or before the event deadline dates will not be accepted.

As requested by the German Shepherd Dog Club of America-Working Dog Association, Inc. (GSDCA-WDA) you will need to read, understand and agree to the Release Statement and Waiver of Liability listed on the entry form and in the chapters below.

• Adult dog handlers must be 18 years or older.

Young dog handlers 8–18 years of age can compete in Schutzhund Trials, Conformation Shows and Breed Surveys according to the GSDCA-WDA Junior Sports Program. At least one parent, guardian or sponsor must be a member of GSDCA-WDA, and the parent or legal guardian must sign all release forms accepting responsibility for the Junior participant.

*) Effective September 1, 2008, in order to be eligible for a V rating at GSDCA-WDA sanctioned shows, dogs born after January 1, 2004 must have an “ED” stamp with the result “normal”, “fast normal”, or “noch zugelassen” in the pedigree.

Step	Instructions
1	Click on the PDF icon or the text link, which will open the entry form file # (Adobe Acrobat 5.0 PDF v1.4 file, 337 kbytes) inside a new web browser window. Please wait until the PDF file is completely loaded. Depending on your computer and Adobe Reader version this could take several seconds up to a minute or so. Adobe Acrobat Reader 5.0 or higher and/or an Acrobat Reader browser plug-in needs to be installed to work properly.
2	Fill in your entry form data into the highlighted form fields and click the appropriate check boxes.
3	Some form fields may not be applicable to you. In such cases leave the fields blank or place “NA” (not applicable) instead. Please make sure that you enter a valid e-mail address.
4	When you are done filling out the entry form proof read all entered data before you print and then sign the entry form.
5	Click the red transmit button at the bottom of the entry form page. Your entered form data will be transmitted to us for processing. It may take a few seconds but then you should see our “Thank You” page. You will also receive an automated confirmation e-mail that we received your entry form data.
6	Complete your package by adding a check payment or money order, payable to Donna S. 1	Download the file # (Adobe Acrobat 5.0 PDF v1.4 file, 337 kbytes) by right mouse clicking on the PDF icon or on the PDF file name link and then select “Save Target as…” to save the PDF to your computer’s hard drive.
2	Start Adobe Acrobat Reader 5.0 or higher, then open the PDF file.
3	Fill in the highlighted form fields with your entry form data and click the appropriate check boxes.
4	Some form fields may not be applicable to your dog. In such cases leave the fields empty or place “NA” (not applicable) instead. Please make sure that your entered e-mail address is valid.
5	When you are done filling out the form print it by using Adobe Acrobat Reader’s print menu. It will NOT be necessary to click the red transmit button which is a function for sending entry form data only.
6	Sign and date the printed entry form.
